To REGISTER: Complete the Registration form, include payment with check made payable to "CHAA", and send to: CHAA, 103 Royal Park Dr. #4E, Fort Lauderdale, FL 33309
 
Paying by credit card? If payment by credit card, contact our bookkeeper, Linda Ward, at 954-303-3428
 
PAYMENT OPTIONS: We offer 3 payment options (prices listed on the registration form): Option 1: Full year payment due by Sept. 3rd (From Sept through May -9 months)
Option 2: Two Half year payments, due by Sept. 3rd, and Jan 10th
Option 3: Quarterly payments (9 weeks) Due by: Sept. 3rd, Nov. 1st, Jan. 10th, and March 14th
